Many Canadian passport applications ask for a guarantor: someone who has known you for a period of time and can confirm your identity. If you cannot find an eligible guarantor, the passport program provides an alternative: the Statutory Declaration in Lieu of Guarantor (form PPTC 132).

What the form is for

The PPTC 132 lets you make a solemn declaration in place of having a guarantor sign your application. Because it is a statutory declaration, it must be signed in front of an authorized official, such as a commissioner for taking affidavits or a notary public.

Before your appointment

  • The PPTC 132 is not available online. Contact the passport program to request a copy, or pick one up at a passport office or Service Canada centre.
  • Read the instructions carefully. They explain when the declaration can be used and what information and references are required.
  • Complete the form, but do not sign it.
  • Bring valid government-issued photo ID.

At your appointment

We will confirm your identity, have you solemnly declare that the information is true, witness your signature and complete the official’s section with our signature and stamp. The process is usually quick once the form is filled out.

A few reminders

  • We cannot act as your guarantor, and we cannot confirm facts about you that we do not personally know. Our role is to administer the declaration.
  • Requirements for children’s applications and renewals can differ. Follow the instructions that come with your specific application.
  • Double-check that the names and dates match your other passport documents.
