If applying within Canada or the United States, your guarantor must:
Be a Canadian citizen who was at least 16 years old when they applied for their own passport.
Hold a valid 5-year or 10-year Canadian passport (or one that has been expired for no more than one year on the day you submit your file).
Have known you personally for at least two (2) years (or, in the case of a child's application, have known the parent/legal guardian for two years and have personal knowledge of the child).
If your network does not include someone who fits this exact description, you must bypass the standard application and request a Form PPTC 132 instead.

To ensure Passport Canada approves your application without administrative delays, your mobile session will follow these exact steps:
Step 1: Obtain the Official Form: The Statutory Declaration in Lieu of Guarantor (Form PPTC 132) is not available for download online. You must pick up a physical copy in person from a Service Canada or Passport Canada office, or contact them directly to have a copy sent to you.
Step 2: Complete the Form (But Do Not Sign): Fill out your personal details, including your 5-year residential and occupational history. Leave the signature lines completely blank. You must also provide the details of two (2) references who are not relatives and have known you for at least two years (Passport Canada will call them to verify your identity).
Step 3: Document & Photo Certification: When our mobile notary arrives, they will verify your identity using your physical ID documents. They will witness your signature, execute the legal declaration, and write the mandatory federal phrasing on the back of your primary passport photo: "This is Exhibit A in support of the applicant's statutory declaration." They will also sign and stamp photocopies of your identification papers if required.
