Statutory Declaration — Free Template

A statutory declaration is a written statement of facts that you solemnly declare to be true. Federal bodies use the form set out in section 41 of the Canada Evidence Act. If a government department gives you its own form, use that instead.

This template is a general starting point, not legal advice. The organization receiving your document may require its own form or specific wording, so check with them first. Do not sign or date it before your appointment: you sign in front of the notary or commissioner.
