Use an affidavit when an organization asks you to state facts under oath or affirmation. Fill in the blanks, keep each fact to its own numbered paragraph, and leave the signature lines blank.
This template is a general starting point, not legal advice. The organization receiving your document may require its own form or specific wording, so check with them first. Do not sign or date it before your appointment: you sign in front of the notary or commissioner.
